Classes Wanted for Middle School

You must complete a separate form for EACH of your Middle School students.
Parents, as we choose which class options to include on the final schedule, we would like your input. Please let us know what are the top 5 choices that you would want for your child. It would be helpful to put them in order of preference, so that we can better decide which classes will be offered during each hour. Note that the class selections are NOT according to any schedule/time slot.
